ABOUT SUNSET MANOR’S SUB-ACUTE UNIT
We are an adult program (18 years and older) specializing in the care of ventilator and tracheostomy dependent patients. Our sub-acute nursing unit offers comprehensive and complex medical care that serves patients recuperating from surgery, strokes, and other conditions once treated solely in hospitals. Our sub-acute unit is highly regarded in the community as we have many success stories of our patients going home, though some do transition to other, more appropriate levels of care, as determined by their personal physician.
We realize that selecting a sub-acute unit for your loved one may be one of the most difficult decisions you will ever have to make, but our team is here to provide care for your loved one and to provide support for you.

Our comprehensive care plans include a full-range of services for both short-term and long-term patients, including but not limited to:

OUR TEAM
The Sub-Acute team is dedicated to our residents’ physical and psycho social needs. Under the direction of our Sub-Acute Unit medical director, our team includes: Physicians/Pulmonary Specialists, Nurses (RNs and LVNs) and CNAs, Respiratory Therapists, Pharmacists, Dietitians, Rehabilitation Therapists (Physical, Occupational, and Speech), Restorative Nursing Assistants, Activity Coordinators, Social Worker and Case Manager.
